Why a first dose is sometimes larger than the rest
Without a loading dose, a drug given at a regular maintenance rate takes roughly five half-lives to build up to its steady-state concentration. For a drug with a long half-life that delay can be days, which is unacceptable when an effect is needed quickly. A loading dose front-loads enough drug to fill the body's distribution space immediately, so the target concentration is reached at the first dose rather than after a slow climb.
The conceptual formula is loading dose equals volume of distribution multiplied by the desired plasma concentration, adjusted for bioavailability when the drug is not given intravenously.
Where loading doses are used and their cautions
Loading regimens are common with drugs such as certain antibiotics, antiarrhythmics, and anticonvulsants where rapid onset changes outcomes. After the load, maintenance dosing simply replaces what is cleared to hold the level steady.
Because a loading dose deliberately delivers a large amount at once, it can raise the risk of concentration-related side effects, so it depends on accurate weight and organ-function estimates.
